The following individuals are eligible to receive a copy of a birth certificate:
- Individual named on the certificate (18+ years old).
- Mother of the individual named on the birth certificate.
- Father of the individual named on the birth certificate (or paternity established).
- Maternal grandparents of the individual named on the certificate.
- Parental grandparents of the individual named on the certificate if father’s name is listed on the birth certificate.
- Any individual presenting GUARDIANSHIP PAPERS of the individual named on the birth certificate.
- Brothers or sisters (18+ years old) of the individual named on the birth certificate.
- Spouse of the individual named on the certificate.
- Son, daughter, or grandchild (18+ years old) of the individual named on the birth certificate.
- Great-grandparents of the individual named on the birth certificate.
