Schedule: Monday - Friday walk-ins 8am - 3:30pm
Tuesday evenings until 6:30pm
Fee: $10 admin fee for uninsured / under-insured Records: A shot record is recommended for a child to have immunizations Parent or Legal Guardian: Children must be accompanied by a parent or legal guardian. If the parent or guardian is not available, an individual over 18 years old may accompany the child and MUST present a note signed by the parent or legal guardian.
